ABSTRACT

Objective:To evaluate the clinical efficacy of modified Liqi-Hewei Decoction combined with conventional western medicine therapy in the treatment of Hp-positive chronic superficial gastritis (CSG). Methods:A total of 96 patients with Hp-positive CSG in the First People’s Hospital of Lianyungang and Changshu No.1 People’s Hospital who met the inclusion criteria between January 2017 and January 2019 were divided into two groups according to the random number table method, with 48 in each group. The control group was treated with conventional western medicine, and study group was additionally given modified Liqi-Hewei Decoction on the basis of control group. Both groups were treated for 8 weeks and followed up for 12 weeks. The TCM symptom score, gastric mucosa and pathological grading were scored before and after treatment. Levels of motilin, gastrin and somatostatin were detected by immunoturbidimetry, and levels of CD3 +, CD4 + and CD8 + were detected by automatic flow cytometry and the ratio of CD4 +/CD8 + was calculated. The Hp negative conversion rate was observed, the adverse reactions were recorded, the clinical efficacy was evaluated and the recurrence rate was statistically analyzed. Results:The Hp negative conversion rate was 89.6% (43/48) in study group and was 72.9% (35/48) in control group, where the difference was statistically significant ( χ2=4.376, P=0.036). The total effective rate was 95.8% (46/48) in study group and was 83.3% (40/48) in control group ( χ2=5.031, P=0.025). After treatment, the scores of epigastric pain, upper abdominal fullness, acid reflux, belching and poor appetite in the study group were significantly lower than those in control group ( t=8.919, 3.971, 7.949, 8.171, 9.865, all Ps<0.01). The scores of gastric mucosa and pathological grading were significantly lower than those in control group ( t=13.705, 15.495, all Ps<0.001). After treatment, the levels of gastrin [(126.15 ± 14.36) ng/L vs. (152.38 ± 17.51) ng/L, t=8.025], motilin [(93.59 ± 11.87) ng/L vs. (102.48 ± 14.68) ng/L, t=3.263] and somatostatin [(36.76 ± 8.97) ng/L vs. (40.84 ± 10.68) ng/L, t=2.027] in the study group were significantly lower than those in the control group ( P<0.01 or P<0.05). The levels of CD3 +, CD4 + and CD4 +/CD8 + were significantly higher than those in the control group ( t=6.883, 6.720, 4.306, all Ps<0.001). The recurrence rate was 4.3% (2/46) in the study group and was 17.5% (7/40) in the control group ( χ2=3.950, P=0.046). During treatment, the incidence rate of adverse reactions was 8.3% (4/48) in the study group and was 12.5% (6/48) in the control group ( χ2=0.446, P=0.504). Conclusion:Modified Liqi-Hewei Decoction combined with conventional western medicine therapy can effectively improve the levels of gastrointestinal hormones and clinical symptoms, enhance the immunity and Hp negative conversion rate, and reduce the recurrence rate of patients with Hp-positive CSG.

ABSTRACT

Objective To explore the surgical procedure for combined heart and kidney transplantation in rats. Methods Inbred Lewis rats were used as donors and recipients. The left kidney and the heart were removed from the donors,the donor kidney was therefore transplanted to the left side. Revascularization of renal artery was constructed end-in-end by modified sleeve anastomosis. Renal veins were anastomosed using stenting technique. Ureters were anastomosed using end-to-end interrupted suture technique. The aortic and pulmonary arteries were anastomosed to the recipient abdominal aorta and inferior vena cavaLewis rats receiving right nephrectomy were assigned to a control group. Results The time of warm ischemia was 10s,and the donor nephrectomy time was (30 ± 3.0)min;the removal of donor heart was(6 ± 1.8)min,the renal cold ischemia time was(40 ± 3.2)min and heart was(70 ± 4.1)min,the meantime for recipients operation was(95 ± 5.4)min,and the graft survival rate was 80.9%. Renal function and heart rate did not differ significantly between the two groups 30 days after the procedure. Conclusions The new model for simultaneous heart and kidney transplantation in rats is feasible and reliable. This technique can be applied to basic research on multiple organ transplantation immunity.

ABSTRACT

Objective To observe the effect of traditional Chinese medicine on proteinuria and inflammatory factors on the model of diabetic kidney disease rats. Methods The male SD rats were divided into the blank group, model group, treatment group, control group. The diabetic kidney disease model was induced by using the left inferior abdominal cavity. After eight weeks, we observed the changes of the rat's weight/kidney weight, blood sugar, IL-6, urine microalbumin, MCP-1 and TGF-β. Results Compared with the model group,the blood glucose(9.96 ±3.73 mmol/L vs.17.16 ±5.75 mmol/L)and Proteinuria(11.73 ±4.01 mg/L vs.19.57 ± 8.38 mg/L,P<0.05) of the treatment group significantly decreased (P<0.05). Compared with the model group,the level of IL-6(58.87 ±14.69 ng/L vs.85.22 ±12.16 ng/L)and the MCP-1(1.52 ±0.86 ng/L vs.3.17 ± 1.26 ng/L)of the treatment group were significantly lower (P<0.05). Conclusions The Chinese medicine can reduce the FPG, proteinuria and the removal of serum inflammatory factors in diabetic kidney disease.

ABSTRACT

Objective To improve arterial anastomosis method for renal transplantation model in rats.Methods Male Fisher and Lewis rats were used as kidney donors and recipients respectively,and left kidneys were harvested in situ.Revascularizations of renal artery were fashioned end-to-end by the modified sleeve anastomosis.The renal artery was placed in the orthotopic position and sutures were inserted in order to place the feeding vessel into the receiving vessel.One invaginating suture was placed starting outside the donor's renal artery wall,approximately 2 mm from the free edge,then passing through the free edge of the recipient renal artery,and lastly passing again through the donor's renal artery wall out alongside the point of entry.The sutures Were tied so that the recipient renal artery was drawn inside the donor's renal artery.Thereafter,one external stitch was placed opposite to the invaginating suture passing through the overlapped free distal edge and the adventitia of the recipient renal artery,and the suture the opposite side using the same technique.The renal veins and ureters were anastomosed using end-to-end interrupted suture technique.Results Twenty cases of rat renal transplantation were performed.The transplantation procedures took totally between 70-90 min.The time for arterial anastomosis was approximately(4.6 ± 0.6)mint the mean time for anastomosis of the renal vein in 20 grafts was(11.8 ± 1,2)min.and ureter was(12.2 ± 1.4)mia The successful rate of the model was 95 % at the 5th day.Conclusion New end-to-end technique which incorporatesa modification of the sleeve anastomosis is the safest way to perform a revascularization of renal artery.This suggests that the technique is feasible and reliable.
